Please note, membership type is for administrative purposes only and a separate Standing Order is required to ensure monthly payment is received by Bradford Hockey Club. Please see payment details section below.

EMERGENCY CONTACT DETAILS

Please ensure you have obtained the consent of your emergency contact before you provide their contact information to the Club. Please also ensure that they agree for BHC to hold their contact information and that they are in agreement with taking on the role of emergency contact.

MEDICAL INFORMATION

Please detail any medical information that we should be aware of, eg epilepsy, asthma, diabetes, etc. Please state any allergies.

If you are taking any regular medication that will affect your participation in Club activities please state what, and for what reason?

Please advise if you have any long-term illnesses, or injuries, that will affect your participation in Club activities?

Declaration:
By completing the membership application process I consider myself/ my child to be physically fit and capable of full participation and agree to notify the club of any changes to the medical information provided. In the event of any injury I give my permission for the team manager, coach, club official or nominated first aider to obtain emergency medical treatment on their behalf

CONSENT (TRANSPORT (JUNIORS)/PHOTOGRAPHY)

By selectiong, Yes, No or N?A please inform us of your consent for teh following:

TRANSPORT:
I consent to my child travelling to venues for matches and training by transport provided by the Club, which may include travelling the private cars of other club members, players or volunteers.

PHOTOGRAPHY:
In some environments, it is impossible to control photography by external parties. I am aware that there may be times that photographs and/or footage may be taken during matches and training sessions by approved agents and/or club officers. Such images shall only be used for publicity/training purposes in accordance with our Safeguarding and Protecting Young People in Hockey Policy.

I hereby give consent for myself/ my child to feature in such images and grant approved agents the right to use the resulting images including any reproductions or adaptations for all general purposes eg local newspapers, other promotional articles and the club website. I will not place photographs of children other than my own on my personal social media without prior permission of their parent.

PAYMENT DETAILS.

I agree to pay fees as stipulated for above at the rates agreed by the Annual General meeting of Bradford Hockey Club for the relevant season. I understand that if fees are not received by Bradford Hockey Club, I may forfeit my membership

SIGNATURE

I understand that the details contained in my membership application:
• may be used to facilitate my/ my child’s participation in activities relating to Bradford Hockey Club
• will only be passed on to any other agencies directly involved in the facilitation and management of Bradford Hockey Club
• will only be kept as long as I/ my child remains involved with Bradford Hockey Club
• will be kept securely and only transferred to coaches and officials through secure networks

I agree that we (parent and player) will abide by the rules and procedures of Bradford Hockey Club as detailed in the BHC (Junior) Handbook..
